Summary

Conducts health assessment interview focused on wound, ostomy, and continence related care-performing a focused physical assessment. Collects data and information relative to the health or situation of the resident with wound, ostomy, and/or continence care needs. Analyzes assessment data to determine actual or potential problems/issues related to wound, ostomy, and/or continence care needs. Develop and Implement plans for wounds, ostomy, and/or continence care that are focused on strategies to attain expected, measurable outcomes. Reviews existing orders and makes recommendations to physicians, managers, and other members of the healthcare team that are focused on healing, maintaining, or supporting the residents wound care goals. Complete weekly wound roads with wound physician and document on wounds weekly using Ekare system. When appropriate-teaches, consuls, and assists families and residents to assume responsibility for prevention of complications as they relate to patients with wound, ostomy, and continence problems. Develop, provide, and evaluate staff education programs, orientation, in-services and continuing education as it relates to wound, ostomy, and continence. May be assigned other duties as appropriate.

Job Qualifications

  • Graduate of accredited school of nursing
  • Current Missouri nursing license in good standing
  • Current CPR/AED certification required
  • Wound certification preferred
  • Minimum two years of varied work experience as a professional nurse and preferably one of the two years in a post-acute setting
  • Must possess critical thinking skills
  • Excellent communication skills to interact verbally with patients, visitors, and staff
  • Excellent time management and organizational skills
  • Ability to stand and walk for long periods of time
  • Ability to lift heavy objects using proper lifting techniques and with assistance as necessary
  • Ability to transfer and move residents who vary with ability to assist weighing 200+lbs. using proper lifting techniques and with assistance as necessary
  • Ability to push and pull equipment weighing in excess of 40lbs. using proper lifting techniques and with assistance as necessary
  • Manual dexterity, coordination and skillful use of hands when working with residents and equipment
  • Ability to perform necessary documentation
  • Visual acuity
  • Must be able to manage a diversity of activities
